Managed care has inexorably suppressed the amount of time that patients remain in hospital. The spectacular response of large-scale, private laboratories, aided by the ubiquitous fax machine, e-mail, the web, and their freedom from having to educate the next generation of pathologists, heightens the expectation among clinicians for a similar performance from academic departments. Residents no longer have as much personal Time as before with "hot" case material.
